It stacks as you parry attacks and it is activated by casting deadly throw or devastate... with the current meta for warrior, it is fairly useless other than for increasing your parry rating in order to help proc whirlwind more often with a 4 warlord/2 fury build.

Just ignore the blade flurry aspect of lighting reflexes unless you are actually equipping deadly throw or devastate... which you should not be doing unless you are using a shield warrior build.
